Mandatory COVID-19 Waivers (Action Required): Curetivity cannot guarantee that each individual attending events organized by Curetivity does not have COVID-19 or fully prevent any individual from becoming exposed to or contracting COVID-19. Attendees will be required to sign an Event Participant Acknowledgment and Release of Liability assuming the risk of exposure to COVID-19 at the 15th Annual Curetivity Golf Invitational & Gala and complete an Illness Screening prior to admittance. Individuals who do not sign the Release or pass the Illness Screening will not be admitted to the event.
